Advance Directives
Advance Directives state your choice for health care treatment if you cannot communicate or make decisions. There are two kind of Advance Directives: Power of Attorney for Healthcare Decisions and Declaration About Life Sustaining Measures.

Power of Attorney for Healthcare Decisions identifies the person that you trust to make healthcare decisions for you when you are unable to do so for yourself.

Declaration About Life Sustaining Measures (Living Will) describes the type of medical treatment you want or don’t want at the end of your life.

When you become a patient, you will be asked if you have an Advance Directive. If you do, please bring it with you to include in your medical record.
