On the other hand, social isolation and a lack of community involvement can lead to poor health outcomes.
How Broadlawns Supports Social and Community Context
At Broadlawns, we believe that health is deeply connected to social networks and community support. We work alongside local organizations to ensure our patients have access to the resources they need to live healthier, more stable lives. Through these partnerships, we connect individuals with services like housing assistance, nutrition programs, job training, and transportation support. Some of our valued partners include:

Food RX Program: Medical providers can "prescribe" fresh fruits and vegetables to patients and their families along with monthly clinic visits for six months. Patients and each member of their family are eligible to receive $30 per month to spend on fresh fruits and vegetables at local stores and farmers' markets.
Health Coaches: We offer health coaching to empower patients to take an active role in their health and well-being. Our Health Coaches work closely with patients to provide support, motivation, and practical education to help them achieve their health goals. Whether it's starting an exercise routine, setting achievable goals, learning about portion sizes, reading food labels, or making healthier food choices (even on a budget), our Health Coaches are here to guide them every step of the way. Broadlawns patients of any age who are interested in making positive health changes are welcome to participate—no referral needed. Through regular communication and personalized guidance, our Health Coaches help individuals build healthy habits for lasting wellness.
Community Vaccination Clinics: Each year, we partner with local nonprofits to host Community Vaccine Clinics throughout the area, reaching underserved individuals and supporting public health. By offering these clinics beyond the walls of our main campus, we provide essential health education, increase local immunization rates, and work to become a trusted partner in our community’s well-being. These initiatives reflect our commitment to accessible healthcare and support for Polk County’s refugee and underserved populations.
During the current flu (fall 2024) season alone, Broadlawns providers have administered over 700 free influenza vaccines to community members of all ages through five pop-up clinics. In August 2024, our Back-to-School Community Vaccine Clinic, in partnership with DSM Refugee Support, ensured that nearly 30 children received their required school immunizations, preventing potential school displacement.
